The PHNE Southeast Vermont Outpatient Program is a behavioral health services program designed to help individuals manage their substance use and mental health disorders. These potentially chronic conditions can significantly impact the way a person thinks, feels, and functions on a daily basis, often resulting in interference with responsibilities and loss of relationships at home, at work, and in many other life areas.
At the PHNE Professional Group, Inc. – Southeast Vermont Outpatient Program, licensed professional clinicians and practitioners partner with each person to identify unique strengths, needs, abilities, and preferences in order to develop an individualized treatment plan that will guide each person to explore areas of challenge, make desired changes, and facilitate healing at the connection of mind, body, and spirit. Outpatient services for behavioral health issues are tailored to meet each patient’s individual needs. The program helps clients regain control of their lives by establishing sobriety, supporting a sustained recovery, and creating support networks.
Patients may participate in group counseling and/or individual therapy as clinically determined according to need and preference. A clinical and person-centered approach is recommended throughout Outpatient treatment. All individuals are assessed by a licensed clinician to further evaluate clinical need. An additional on-site referral can be made related to any mental health needs, and Medication-Assisted Treatment (MAT) is available in conjunction with treatment services.
